| Peyronies Disease
Introduction
Peyronies disease is an uncommon sexual
dysfunction that makes to having sex difficult and painful
due to bent penis during erection. It is characterized
by a hard, fibrous layer of scar tissue that normally
develops under the skin or the upper or the lower side
of the penis so as it gets erected the scar tissue pulls
the affected area off at an angle gives rise to the
curved penis. Francois de la Peyronie, a French surgeon
was the first to have noticed the disease in the year
1743.
Indications
The indications of Peyronies disease
may develop more slowly or might appear overnight. These
may include:-
- Painful erection.
- A bend or curve in your penis during erection.
- Inflammation under the skin of the pennies that
develops into hardened scar tissue.
- Narrowing of the diameter of the pennies during
erection.
- Impaired ability to obtain an erection (erectile
dysfunction).
Risk Factors
Peyronies disease is indeed an uncommon disorder, yet
some factors are likely to increase the risk.
- Age:- Aging may diminish the penile
elasticity and increase the chance of injury and the
development of Peyronies disease.
- Heredity:- Although heredity is not
the only factor responsible for the determination
of Peyronies disease , yet it plays an important role
in determining the one who is likely to be affected
by it.
- Conditions involving hardened tissues:-
Some men with Peyronies disease experience hardened
tissue in other areas of the body, such as the hands
or the feet. A cod like thickening across the palm
the finger to pull inward known to be Dupuyten’s
contracture may also be associated with Peyronies
disease.
Complications
The complications involved with the Peyronies disease
can be immense as it can make intercourse from being
painful and difficult to almost impossible. Peyronies
disease can have strong psychological impact as it degrades
the morale and the self esteem of the male partner.
In addition to this it adds to the emotional distress
that puts strain on the married life or the relationship
and makes attempt at sexual activity with your partner
stressful and frustrating.
Peyronies Treatment
A lot of medications have been tried out for the treatment
of Peyronies. The most prevalent ones being Vitamin
E, Potaba, Colchicine, Verapamil and Collagenase. Apart
with these treatments, surgical methods can also be
very handy for people suffering from Peyronies disorder.
One needs to fulfill the basic criteria before opting
for the surgical treatment as additional actions are
possible only when bending or deformity is severe enough
to seriously interfere with sexual function or intercourse.
Lot of variation has been used for the treatment of
Peyronie's disease. It has shown positive results in
some patients but for the mejority the medication seems
to make minor difference. Difference in treatment outcome
and the resulting lack of consensus regarding the drug(s)
of choice reflects in part an uncertainty with strategy.
While doctors can't say with precision what causes
the disorder in each individual case, there does seem
to be a common disease-producing process in all men.
The major question of concern is,where, in this complex
spectrum extending from early wound healing to late
scar formation, do we intervene with medications?
Available Medication for the treatment of Peyronies:
- POTABA: or Potassium para amino
benzoate is designated by the FDA as "possibly
effective". Large doses are required, which may
cause intestinal upset.
- VITAMIN E: It has been used for
the treatment of scars, and has been employed in the
treatment of Peyronie's disease since 1945.
- VERAPAMIL: is usually given by
direct injection into the plaque. It also interferes
with the synthesis of scar tissue precursors.
- COLLAGENASE: is an investigational
drug that enzymatically digests scar.
- COLCHICINE: A popular medication
for inflammation, it interferes with the manufacture
of scar tissue.
